Survivorship Care Models at DFCI:
  • Adult Survivorship Clinic: We have 2 full-time Nurse Practitioners, 1 part-time Physician Assistant, and 1 part-time Onco-Generalist physician who provide initial and ongoing survivorship care both in the Yawkey 5th floor clinic and now at Chestnut Hill.
  • Embedded Survivorship Care: We are rolling-out Embedded Survivorship Care to the Disease Centers. In the embedded care model, you would have your first survivorship visit with your own NP/PA from your oncology treatment team. Many patients feel more comfortable in transitioning to survivorship by having their initial experience with the oncology team that has been treating their cancer.
